Ah, but they are not exactly the same, their viscosity varies in units of
centipoises. While the “Gel Tears” is a fine better than nothing, my personal
preference is for the Systane Gel Lubricant as its propensity to bubble for
immersion ultrasound is less. I have to work harder to keep the bubbles out of
Gel Tears vs Lubricants.
Alcon’s Systane Lubricant Eye Gel was the perfect product in my opinion. The
NIH did a big study on the subtle differences, including Ph levels. Their
report went with Refresh Liquigel as the winner.
Ah, what a vast science eye lubricants is!

GenTeal Tears Gel and GenTeal Gel are the same thing. Our latest batch says
"Tears Gel" and we can't tell any difference. Works well even for standing
off the cornea for B-scan biometry.
Regular ultrasound gels can cause corneal irritation. I called one of the
manufacturers a long time ago to find out what was in it to cause that, and was
told it was the pH that was the problem, not what was in it. He said with any
ophthalmic solution they have to be very careful about the pH balance or there
will be corneal PEK. I've never heard of trying tegaderm. Keep in mind,
too, that when going over the closed lid there is an absorption factor that
will prevent visualization of some of the weaker signals (vitreous opacities,
etc), and it's very hard to know where you are in the eye since you can't be
certain of the gazes for probe positioning and marker placement.
You could use methylcellulose in a pinch, but it's too runny and some of your
patients will be allergic to the BAK preservative in it, plus you have to rinse
it out afterward. When GenTeal and Systane gels were backordered, we found
Systane Gel Drops to be thick enough for regular B-scans, but not thick enough
for B-scan biometry. Much preferred over methylcellulose, and like with the
gels, we didn't have to rinse it out.
