Survival is a game mode that both ZDaemon and Skulltag have, and it would be
nice to have in odamex as well.

At a very basic level, Survival is Coop with limited lives.  Once you die X
times, you sit out the rest of the map until it is won (advances to the next
map) or everyone runs out of lives (map resets).

In addition, an enhancement that I think would be worthwhile would be an option
to allow all items and powerups to be picked up once per player per attempt at
the map.  This would allow larger numbers of players in Survival without
resorting to cheap and potentially exploitable measures like respawning items.
