Suzanne,

How do you identify patients in the office?  It may take a little patience,
but it would probably be worth it to use the same identifier in your
database as you do when the patient comes in.  If it is the ssn, you start
running into compliance issues and you don't necessarily want to be using
Access.  As far as normalizing your existing data, your best bet is to start
by getting your patient info into a new table.  You can add your unique
identifier later, just use an autonumber to start.  Get your patient info
in, add your unique identifier and dump the autonumber.  Next you will want
to work on getting your existing data into a new table for transactions.
The list can help with that when you get to that stage.  If you need help
