Roll on the day that happens. I only hope that when it does Microsoft give 
decent support for any problems with it. Although I appreciate that if 
Microsoft did incorporate a screenreader worth the name into Windows 
screenreader manufacturers would complain, but that's not my concern. 
Wouldn't it be nice just to pay for one product instead of several, and not 
to be expected to fork out hundreds of pounds just to enable us to access 
what is accessible to the sighted user.
  Linux have done it, and so have Macintosh. Why not Microsoft?

I believe the market for screen readers is comparatively small so it will
not be at the top of MS's priority list.  Also MS has recently tried to
improve it's security in Vista only to have companies currently offering
third party security products complaining of unfair competition.  I can't
imagine screen reader companies being too happy if MS attempts to take away
their business.  MS has done this in the past of course when it wanted to
get into the word processor market and deliberately crushed a rival word
processor product by giving them very little information on how windows
would work at the time they introduced windows three point one.  This meant
there rivals product was crap in comparison to MS's.

You can expect MS to take an interest in controlling the screen reader
market when they have taken over everything else.
