If you are actually publishing to a TServer inside through the ISA
box, 
ensure that
Remote Desktop is not enabled on the ISA box itself, as the port
(3389 by 
default)
will be in use by the ISA box.   Create a custom protocol definition
for 
inbound tcp
3389, and create the publishing rule with that.
