Users can only change the password once in OWA

Hi all,
My collegue installed the change password functionality in OWA but for
unclear reasons the password can only be changed once per user. When a
user tries it for the second time we do see the login sequence and all
in the logs (no errors are reported) but OWA says that it was unable to
change the password.
Has anyone here ever seen this kind of behavior before and how did you
resolve this.
If not can anyone give some hints on how to further debug this?
