Steven, the first thing to check would be whether you've overridden the
custom-icons path. There's a setting for that on the interface page of the
config dialog. If set, it will stay in your config data until changed or
reverted to default.
If that's not the issue, next check what happens when you click on a custom
icon (or in your case, a missing icon) on any toolbar's config page. There's a
directory-selector there.
