Are you using the index as your main way of navigating your pages, or
some other method?  The index will always show the "real" name with
underscores.

The method I'm talking about is that if you have  the following files:
bombs.txt
fat_man.txt
little_boy.txt

bombs.txt looks like this:
===== Bombs with interesting names =====
[[fat_man]] - big ba-da-boom!
[[fat man]] - another link to the same file.
[[little boy]] - not so big bang, but still more than I want to see.


The first one will show the text in the link with the underline and
the second will have a space, but they will link to the approriate
files.

You can just start changing the way you create your links and update
your files as you go.

BTW, don't ask where the bombs example came from, I'm feeling a little
under the weather today.  :-)
