Tested on a current DW install, it is working. The "return
$id:$conf['start']" page if exists is cleaner than in my previous
patch. Pagemove is not broken. For me it's ok. breadcrumbs part of
your patch kind of mimics youarehere behaviour. pagesearch part WFM.
As suggested in my private answer, please test it with userewrite
and useslash on/off.

Always patch against current- if you want people to test your work
and make comments.
