Hi Gary,
Indices are not considered core content of books, so most validators and 
scanners don't edit them. They usually don't scan well, and the thinking 
usually goes that if the reader needs it he/she can do as well at sorting it 
out as the scanner/validator could. Maybe in the future this will be different.
