the entry and start of the text. That looks the neatest, I think.

I'd suggest going with Misha's first inclination and place 3 asterisks
between the end of the catalog entry and the beginning of the story. The
asterisks would clarify that there is a change in focus.

nothing should be added. Asterisks are sometimes added to substitute for
blank lines that show where a break in focus occurs, but the bookshare
conversion tools remove blank lines so the asterisks are needed as a
substitute. If there is no blank line or other indicator in the
original... then add nothing odd as it sounds, seems appropriate.

beginning of each chapter. It is directly under the chapter heading and
