From an IT Perspective, I would make sure your cable connections and power
connectors to the floppy drive are good.
Also, floppy drives do die... I wouldn't rule that out. You can get a new one
at a local computer shop for $15-20 and try plugging it in without installing
it to see if it consistently comes up when starting the computer.
