Yep it's been happening under all the versions of iOS 6 I think. I've
probably had it about 3 times of about 10 hard resets. I don't do them
anymore unless I know I have someone handy to turn it back on just in
case.

sighted person going into settings and turning it back on for you. All
your Voice Over settings will have been returned to defaults and the
triple tap option will need to be set back to toggling Voice Over.
