Thanks for this. That's a shame about iOS. Is there anywhere that we can
submit requests for improvements for future updates?

Under iOS, no. Under OS X, yes. Open VoiceOver Utility with
control-option-f8, interact with the categories table, find speech, stop
interacting, control-option-left/right until you find pronunciation tab,
press vo-space to select. Unlike Windows you have to select tabs by
activating them.
