[TCUG] High Signal Heads

On two sites we have signals mounted on high poles which have come loose.
On the first site the signals had been installed for approximately two weeks
when a secondary high mounted signal head completely fell off, at the time
we put this down to possible installation failure with the bolts not be
properly tightened. On site two an existing installation where we replaced
existing high signals with LED we have had two signal heads come away from
the top bracket. This makes two occasions with the same manufacturer, we are
therefore concerned that this is a design issue which could affect using
this particular signal head again. My question is, has anybody else suffered
problems with high mounted signal heads coming loose?
I have purposely not mentioned the manufacturer whilst the cause is still
being identified, if you wish to give me any instances with a specific
manufacturer I would suggest you send them to me directly using the email
below.
