There's a Bond movie where the camera gets caught in the mirror
during a fight scene.

shots. They are more common than you would think. Usually something small,
a cigarette smoked half way in one shot becomes a full one in the next.
Script girl is supposed to catch this stuff but who knows how many shots
are between two that are adjacent in the finished movie.
