I’m inquiring about the suffix (numeric code) that hospitals assign to residents and other prescribers that is intended to be used in conjunction with the institutional DEA number. 1. How many integers in the suffix? 2. Does it contain both number and letters? 3. Is the suffix recycled and if so how often? 4.
