1. How many integers in the suffix? 3

2. Does it contain both number and letters? Numbers only

3. Is the suffix recycled and if so how often? Yes, every few years

4. Have you considered using the employee's ID number as the suffix which
many be as long as 7 digits. No

Subject: [nyschp.dop] Re: List Serve In Full Operation

Dear Listserv Members,

Fred Cassera, Director of Pharmacy at Maimonides Medical Center has the
following question for the listserv:

With the advent of e-prescribing, there are some issues that are arising that
need to be addressed and perhaps standardized. I'm inquiring about the suffix
(numeric code) that hospitals assign to residents and other prescribers that is
intended to be used in conjunction with the institutional DEA number.

5. How many integers in the suffix?

6. Does it contain both number and letters?

7. Is the suffix recycled and if so how often?

8. Have you considered using the employee's ID number as the suffix which
many be as long as 7 digits.

Please respond through the listserv.

Thank you
