We use Hospira or Baxter for the floors and reserve Braun for the phcy

How are other organizations dealing with the unavailability of  Dextrose 5 % 
and Normal Saline 250ml bags:

Decanting from a large volume into empty 250 ml bags Change to different 
diluent, when possible Change IVPBs to a different  volume Other

At this time we do not use suffixes.  Our housestaff have to get the Attending 
Physician to prescribe controlled substances.  I don't know if this will 
continue into e-prescribing, and I certainly have discussed the issues with 
them if they choose to continue this practice, but to date I have no response 
that they will change this practice.

2.      Does it contain both number and letters?  Numbers only

3.      Is the suffix  recycled and if so how often?  Yes,  every few years

4.      Have you considered using the employee's ID number as the suffix which 
many be as long as 7 digits. No

With the advent of e-prescribing, there are some issues that are arising that 
need to be addressed and perhaps standardized.  I'm inquiring about the suffix 
(numeric code) that hospitals assign to residents and other prescribers that is 
intended to be used in conjunction with the institutional  DEA number.
