It's not IP address authentication, it's password authentication. Otherwise you got it right except that some are POP3 authenticators and others are SMTP authenticators. SMTP authentication means it has to log on (authenticate) before it sends, regardless of whether or not you've checked for mail. POP3 authentication means it signs in to get mail, and then because you logged in to get mail, you're already authenticated to send for a period of time.
