Hey Mark,
I am on the very latest update for OS, and I have tried your suggested methods
for turning on/off speech but nothing works unless I disconnect the headphones
altogether and reconnect them.

This almost always happens with new updates. When some one says my system
is working in many cases that simply means that they are not using some of
the same software. I guess the only thing that you can do is to make sure
that your apps are up to date and hold on until late summer. By that time
Apple should get there act together, grin, smile, wink.

no longer works as smoothly with most applications as it did before. I am
also noticing that using headphones voiceover often does not read to me so
