Yet you can manually start a program like Narator from the login
screen. You'd think it'd be the same thing.

When you're at the login prompt, you are not a specific user, so you are not using a psecific desktop, and as a result, no hotkeys have been defined. It's a security measure. The whole idea of the login prompt is that you shouldn't be able to manually start any programs before you're logged in.
