Actually, you should be able to use the program's title in the Start Menu to
create a hot key as well.  I just tested this in Windows XP by creating a
hot key for iTunes.  After locating iTunes in its subMenu, I used the
Applications Key to bring up a context menu with Properties as one of the
choices.  Shift+F10 will also work to bring up the context menu.  In the
Properties dialog, I pressed Control+Tab to get to the Shortcut page where I
typed i into the Shortcut field, tabbed to the Run combo Box to select run
maximized and then pressed Enter.  Control and Alt were added to the
shortcut key by Windows automatically.
