There is a simpler solution. One should be able
to use BRoster information on which application/
window gets activated when selecting the window
one wants to annotate.

You have to initiate the annotation somehow, by
using a utility application ("Add note to a window"?)
by pressing a SpicyKeys key combination, clicking
its replicant in Deskbar or opening it from Deskbar
or Tracker. Any of these should work.

This application could ask you to click the window
you want to annotate, wait and see which application
gets activated (receives focus), check which window
is the active one, and then open a pad on top of it.
