FineReader - use it in work situation all the time.

For home use, I purchased OCR software from AABBY company. This software
included semi-ocr ability to take PDF files and turn them into Word files. 
I have used this to produce about a dozen pdf files into braille that came
out in decent quality. Since there are many tools and approaches to making
PDF files, there is no way to say how a particular one might turn out. I
would be wary of any file that displayed in columns.
