I'd prefer a button, but am okay with a checkbox as well.
Checkbox or image button ... I can't tell as long as I haven't seen both
variants (because both could be done well or badly ;-) ). But I am sure
both are able to look good and be functional and intuitive!
(One advantage of an image button would be, you wouldn't need the save
button anymore.)
Another question: How many of the three buttons really need to be
buttons? I suppose, the info button could be a link (e.g. on the plugin
title)?

Yes, I think so.
A question for you ... the configuration settings aren't really a
table. Do you think its worth restructuring things to use a list?
Hmm, good question ... I think, the table is fine. (You could even label
invisible <th>s with "Setting type" and "Setting value" or similar.)
The alternative of having no table at all (and just styling labels and
inputs to look like a table) would be more semantic, of course.
But I don't think it's worth it ...
Forms are a weak point of DokuWiki, anyway (from the html+css point of
view!) ...
