We are still working on the issue, but if you select the link that says
Downloaded in your My History page, sometimes you can get a book to download.
I know that's not much, but hope it helps some.

having been downloaded; but there is no way to initiate a re-download, nor is
there any evidence whatsoever that this book ever did download successfully

In fact, I am convinced it has not downloaded even now.
The alt+s command does nothing for me either, even after waiting a
minute after hitting the Download button connected with my desired
title. Emptying my cache of temporary files and cookies before a
download attempt effects no positive change, either.
