I am pretty sure her email address is in the comments of her submissions.  It 
is not uncommon for people to forget to look there, unfortunately.  I know my 
problems were over the holidays, but while I was not getting replies at that 
time, there were more submissions being made.  Later, when she reached me, she 
said she had computer problems, but at that time, I had no ideas the book had 
more problems than chapter headings.  It wasn't until I had a hard copy to add 
Chapter headings I saw how many more problems there were.

Sorry I stirred up a hornets nest; I wanted to be proactive, not destructive, 
and am limited with my computer time right now.  My bad for the way I handled 
it; I did not want others to slug through tough issues, especially if they 
might not have a book to refer to to get things right.
