Good point, Tom. That's probably why, when chapter
numbers are given instead of titles and without the
word chapter, they are usually, if not always, in a
different size and type of font, i.e., bold or
ital--for the sighted. So indicating a chapter with
the word chapter, in my mind, doesn't change the
author's intent. Besided, it's probably the
publisher's decision, not the author's.
