Thanks to you both for your posts.  Marie, it was interesting to see the format 
of the form.  I'll keep it for reference.

George, the basic situation is thus.  A product is usually ordered on my 
behalf, with the purchaser submitting the VAT form following receipt (and 
payment) of the goods.  However, seeking authorisation for the purchase 
requires validation by two or more persons and can be a timely process.

In this case, I had permission from the usual purchaser to order the product 
myself, make payment myself and sort out the VAT reclaim myself.  I will then 
be reimbursed the required amount from the usual purchaser.  This was done to 
save time as the product was needed rather urgently.

The situation as it stands now is that I have received the product and made 
payment for the full amount - the entity with whom I am dealing have never to 
my knowledge deducted VAT prior to purchase.  So, I now require to claim the 
VAT back, with my concern being which form I should use given the poor quality 
of the original sent to me, and to whom it should be sent (the seller or the 
Inland Revenue?)

Eligibility is not in question as the product has been being ordered as 
eligible for the last 12 years.  Running a business myself as I do, although 
admittedly not catering to the disabled directly, I would hope I would never 
file any tax or other legal claims with fraudulent intent, although mistakes 
can happen and for that I thank you sincerely for your insight.

Thanks for your continued help.  Hopefully this clarifies the position somewhat.
