[access-uk] UK VAT reclaim form for items purchased for the blind

Hello,
I appreciate this isn't strictly technical, but I was wondering if anyone
knew a link to download the form used to claim back VAT on items purchased
which were manufactured for use by the blind? I have an eligible purchase
and receipt, but the form I was sent is of such poor quality I'm struggling
to read it even under a CCTV.
Please note the form is for use with UK authorities for a purchase made
wholly within the United Kingdom.
Thanks for your help. It's the first time I've done this so I'm a little
unsure at this point.
