Yes I have seen this. Was a bloody nightmare. I assume you are doing a scripted install of windows. There is an updated ntfs.sys contained in one of those hotfixes for SP4 that screws things. I believe you only see this if you have done a resize of the NTFS partition during the scripted install using the SP4 media and then update hotfixes. MS have released a post hotfix hotfix that fixes it. Can't remember exactly what hotfix this is in but do a search for updated ntfs.sys. Also I think this shows that you should only use hotfixes when you have an explicit problem. Downloading unknown hotfixes form a web site without testing thoroughly is asking for trouble.
