[THIN] Hosted desktop provider

Greetings everyone. I am looking for some providers to look at that provide
full hosted desktop. This is for SMB use, 3-5 users.
It can be Citrix, TS, VDI, doesn't matter right now. The one catch is, the
company needs to be USA based. If you know if a provider you are recommending
will allow "custom apps" (ie. QuickBooks Enterprise + the database server),
please let me know.
