Our clinical application (Epic) locks the screen after idle time out, so Hipaa requirements are met at the application level. We have PC's in exam rooms with the application up and running throughout the day, sometimes idle for quite some time. HIPPA creates some issues; you are supposed to enforce some sort of lockout for when an operator leaves a station in a public area unattended to prevent access. HOwever, if you establish a policy which requires operators to lock their workstations, and some method for auditing this, you can probably use pretty long session idle timeouts (8 hours is what I use). Disconnect timeouts for me are mostly a housekeeping issue; I set them in at 15 minutes, with overrides for some users who do large bulk processes.
