[TechAssist] help CTC187CL2 blows H Out transistor

Hello Techs,
This unit came in with Q4401 shorted, it lasted a day and shorted again.
I relaced the flyback T4401 and returned to the customer. A week later it
failed again Q4401 shorted. Has anyone seen this repeat failure? What is the
most common problem?
