Do any of your sites have plans for the roll out of the new POLST form?
The Physician Orders for Life-Sustaining Treatment (POLST) Documentation
Committee has presented its proposed changes to the POLST form to the
Emergency Medical Services Authority, and the changes have been approved to
take effect Oct. 1. The form updates are focused primarily in Section B and
were developed by the POLST Physician Leadership Council to clarify and
help patients better understand treatment options. To preview the 2014
