My advice is to take the dog to the vet and have the surgery, but get a second
opinion on the cost. I am wondering why this person would let the dog suffer
with the pressure and pain in the ear for 6 weeks without any surgery to
correct it?
My girl had an ear hematoma several months ago. I just came home one day after
work and her left ear was thick and spongy and if was bothering her. My vet
drained the blood and closed the blood vessel (laser/stitch I think). She put
about a 5" tube in her ear that was stitched in and it drained the fluid/blood
out and it had to stay there for about 10 days, then removed. She has a little
thickness, but her ear is ok now. The ear must be drained and something has to
be done to stop the blood vessel from leaking, otherwise you will have
cauliflower ear.

We aren't sure what happened to her, but it is usually caused by some trauma to
the ear. Could be something that hit her ear or from her scratching it really
hard with her toenails. One of my other dogs plays really rough with her and
he may have hurt her that way. He also had been playing with a new toy that
had a hard ball inside a pull toy. I have since thrown it away. I can't
recall the exact cost for the surgery, but I think it was around $300.

Hope this helps.
