It certainly looks like a maiden name, and there have been many other similar
wordings, so index it as a maiden name.

This notice has a name in brackets after the forenames, but it smacks of being
a maiden name, although its not in the correct place.
Should I treat it as a nickname of takeas as an alternate surname.
I have checked existing entries and there is only one from a different paper,
that doesn't mention this alternate
