I should have probably re-posted, but I figured out the problems.
Ultimately it was permissions. When I unpacked the initial package, I didn't
copy over /gateways folder with the installation, so when I did it after the
fact, I had to go back and do the following:
sudo chmod -R 0755 /etc/var/www/html/racktables/gateways
AND...
sudo chown -R apache:apache /etc/var/www/html/racktables/gateways
I did the group change first, and that allowed some things to finally display,
and then when I set the permissions properly, voila!

I got to a stage where it no longer gives any errors but all the
ios12.connector.xxxxxx files in /tmp are still blank and all I get returned in
the browser are the headers and a checkbox so still something not happening for
me.

NOTE: I didn't have the "gateways" directory in /var/www/html/racktables

apache:apache on the folder and it's contents, as well as ensure the
permissions were set to 0755 to enable the PHP scripts to execute. I pulled
this information from another post and am hoping it's right. Additionally, I
did edit my /gateways/deviceconfig/switch.secrets.php file to include:
Devicename telnet 172.16.xxx.xxx - - - telnetpass enablepass
Also tried...
Deviceip telnet - - - telnetpass enablepass
Both of those examples I pulled from other List Archived posts.
Now, I've tried parsing through the list archives and can't seem to find
anything immediately or readily helpful. Please be patient with me as I don't
know any PHP, and I'm fairly new to Linux in general. I will provide anything
you need for debug output, but you may have to give me a little help along the
way if you don't mind.
