This is indeed important.  You are right, Mark, the first $100 of coverage
is free; it's something like $1 or $2 per additional $100 of coverage.

On the airbill, the sender determines the replacement value of the item, 
with a general description. I usually enter "framed artwork" or "framed 
photograph."

I have had to make claims on at least 2 occasions that I can remember.

Depending on the carrier, it can take between immediately (UPS) to 
8 weeks (DHL).  There is a brief form to fill out describing the damage.  
They have never asked for proof (I guess they figure it's cheaper to just 
pay than spend the manpower to investigate), nor have they ever 
contested my claims.

I strongly recommend UPS over DHL.  I recently had a horrible experience
w/DHL; they are a badly-run company with layers of badly-trained
employees, and non-consumer-friendly policies and procedures.
