Might check the corner of the press where the loose 11x14 tag was sitting 
during the pressing.. could you have an element that went out?  I suppose if 
that corner were wet, that would have hampered adhesion too.

Both of these causes would have 'disappeared' when you mounted the 11x14 the 
second time, assuming you put the bad corner at a different place on the 
press platen.
