Debian includes speakup and brltty in some of the official CD images.  See
this web page for instructions on where to download the daily built images
and how to install with speech or Braille.

Speakup, orca and a lot of the other screen readers are in the debian
repository.  So you can use aptitude to install them or keep them updated if
they are already installed.
