In terms of .net, I wish I could say that SharpDevelop was accessible.  Though 
it's written in WPF, which is generally accessible, it's not, and none of the 
current developers on that project are even remotely caring enough to even 
think about it.  It will take one of us (probably me eventually as for my new 
adventures in C#).  WPF's great, but not when we've got devs using it who don't 
care about us.

While people may still be using VB 6 somewhere out there, Visual Studio Express 
is free, up to date and there are scripts for JAWS available for it.

There are also other development tools out there and you can find lots of 
traffic about them on this list.
