Just a random thought but did you check the NVDA log at all? See if the
addon is getting loaded at all?

Tested/tried it out under firefox 67.0.
There's nothing showing up under the input gestures dialogue, which I
would expect to render these commands, and, FWIW, I do also have, for
example, the mozilla apps enhancements add-on installed, in case relevant.

This is on windows 10 64 bit 1903 - sure not relevant, but, thought

* Pressing a shortcut key will bring up a detailed report of the
control in a virtual buffer. At the moment, it only shows the
focused object's size and position. Other information will be added
in future updates.
* NVDA will announce a message indicating that navigation in the DOM
is not possible when outside of the page content. Such places
include Firefox menus and other dialogs.
