My understanding of what you want to achieve, is that you want Phorm to log 
data to a database, e-mail an autoresponder to confirm reciept of the data to 
the user, and then have some way to later retrieve that data from the 
database. 

Starting with acknowledging the user, you can set up an ack page or 
redirection that takes the user to a specific page after they submit the page. 
Redirect will go to a static form that tells them you have the data. 

The ack page can be used in a similar way but you can display the information 
the user entered. To do this enclose the field name in double parenthesis 
{{FIELD_NAME}} and Phorm will convert it to the actual data. 

Sending the user an e-mail, follow the directions in the Phorm Documentation 
for sending an autoresponder. Ensure that the field on the form where the user 
enters his or her e-mail address has the attribute name="PHORM_FROM". The 
autoresponder will not be sent if they don't enter an e-mail address. 

Do NOT use the E-mailing Form Contents section - this is where you set the 
e-mail to goto you if you want. 

To log to a database, use the Logging to a MySQL Database section of the Phorm 
Documentation which contains quite detailed instructions on database logging. 

The tricky one is to retrieve the data from the database. If you have are 
familiar with PHP you can use PHP to extract the information and display it in 
a webpage - if that sounds too hard, I suggest something like phpMyAdmin, 
which is a MySQL database management tool. phpMyAdmin is available as freeware 
from:
