I don't get any errors when I submit data on the form, so I 
can't tell what's happening.  I submit it, and the inputted 
data vanishes.

I don't know PHP, I only know HTML.  Try asking about this at 
http://tech.dir.groups.yahoo.com/group/HTML-Haven/?v=1&t=directory&ch=web&pub=groups&sec=dir&slk=3
 
it's free to join.  I've been a member there for over 10 years.

If it WAS working, then I would first ask your host since they 
may have changed something, like the PHP version for example 
that may not be compatible.

fill in the form, doesn't matter if the data is valid or not,
because it won't go anywhere.

And, that's the problem.  This form used to work but in
recent weeks it has stopped working.  From what I am able to
figure out from searching for an answer is that there must be
some kind of PHP configuration file(s) that is missing or
incorrect on the hosting server for this website.

When the form did work, there would be a reply saying, "Thank
you for contacting us.  Your message has been sent
successfully.  Click here to continue."  If there was a
missing field it would tell you that too.  Now, the only
thing that happens is that the blank form just keeps coming
up.  The submit button doesn't seem to be recognized.
