That will work for database logins. I’m more interested in making 2-factor
work for application logins for an app running on Oracle Forms in a WebLogic
server.
Does Oracle even have an off-the-shelf solution for this, or are we going to
end up writing our own new authentication procedures.

Subject: Re: 2 factor authentication for Oracle Forms
Hi Matt,
I've done it with 12.1.0.2 Standard Edition, Freeradius, and Google
Authenticator.
I've been meaning to blog about it, but I've been lazy.
Note that if you use 12.1.0.2, it works without ASO and so also works with SE.
Before that, it requires ASO.

I’ve been asked to some up with possible suggestions for enabling two factor
authentication for our custom written Oracle Forms application.
So far, the only solution I’ve really seen is using Oracle Access Manager to
handle authentication, which would allow 2 factor (probably using Duo which we
already have in place for 2 factor authentication to the VPN).
What other solutions are you aware of?
Thanks
Matt
