You can call or email them and they will email the form to you. (That
is correct that this particular form is not on their website.) You
fill it out and email it back. They email you a pdf of the permit,
which you print and have in your car. I recommend calling rather than
emailing for the initial contact--I emailed first and never heard back
from them.
That is one way to get the permit.
You can also visit their office to get one, during normal business
hours as seen on the website. The address is on their website. It's in
town, at least a couple of miles from the ponds. I would call first to
be sure someone will be there. Philomath is not a big place and I've
shown up there to find a "Back in an hour" sign (or something to that
effect).
So... start the process by calling them!

page doesn't seem to be applicable to birding, nor do I see anything obvious
