is your modem disabled under Network in System prefs?
for some reason your Mac is not seeing your modem (in Leopard, you can
select it in the services list , click the gear icon and choose Make
Service Active)
the only other (drastic) solution I can think of is perhaps an archive
and install of the OS (what version are you running?)
does your modem work otherwise?
