[mso] Help with fields in WORD

I have a form that I want to lock an area like:
Name
Address
Phone Number
I don't want these fields able to be changed or altered the words Name,
Address etc).
I need help on setting a basic form that I can tab to, input the info
and tab to the next section.
Can anyone shed some light on how I can do this?
